I have received a copy of the Code of Conduct, and it is my responsibility to read and abide by the policies and procedures contained therein. I agree that if there is any policy or provision in the Code of Conduct that I do not understand, I will seek clarification.

I understand that anyone associated with the league or in attendance at a league-organized event found in violation of the code of conduct or the values expressed in the mission statement, the statements on justice, equity, diversity, and inclusion, or the equal opportunity statement may be removed from the league or league-organized event for the safety of others, subject to disciplinary actions, and/or required to take part in restorative justice and/or remedial action prior to further league participation.

I understand that participants of the league and league-organized events are not contracted for any specified period of time, and that I am therefore free to resign from the league or league-organized event(s) at any time and for any reason, with or without cause, just as MCRD is free to end its relationship with me or to change my status (e.g., discipline me, change my ranking, suspend me, etc.) at any time and for any reason, with or without cause.

I agree that this signature page acknowledging my receipt of the Code of Conduct contains the entire understanding between MCRD regarding my participation and status. I acknowledge that nothing in the Code of Conduct I have received is intended to modify this understanding. I further acknowledge that no one other than the league's Board of Directors may modify this understanding and that any such modification must be in writing and signed by two Board Members. I understand that nothing else contained in the Code of Conduct may be construed as creating a promise of league membership, status, or benefits or a binding contract with the league.

I also understand that these policies and procedures are periodically evaluated and may be amended, modified or terminated by the league's Board of Directors. Any exceptions to any policies contained in this Code of Conduct can only be made in writing by the Board of Directors. The current edition of the Code of Conduct and the contents therein, supersede all previous editions, policies and procedures. I accept these policies and procedures as the basis for my participation with the league, as witnessed by my signature below.

Waiver of Liability and Hold Harmless Agreement

I, the undersigned, IN CONSIDERATION for allowing me to engage in, perform and skate during such events hosted by Mission City Roller Derby AGREE TO RELEASE all said landowners, Mission City Roller Derby, its officers, servants, agents, employees, owners or volunteers (hereinafter referred to as Releases) for any and all liability, claims, demands,  actions, and by me, or to any property belonging to me, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASES, or otherwise, while engaged in said activity.

I further grant the Released Parties the right to photograph and/or videotape and further display, use and/or otherwise exploit name, face, likeness, voice, and appearance forever and throughout the world, in all media, whether now known or hereafter devised, throughout the universe in perpetuity (including, without limitation, in online webcasts, television, motion pictures, films, newspapers, and magazines) and in all forms including, without limitation, publication of Event results and standings, without compensation, reservation or limitation. The Released Parties are, however, under no obligation to exercise any rights granted herein.

I am fully aware of risks and hazards connected with engaging and participating in said activity. I understand that there are risks associated with said activity. I hereby elect to voluntarily participate in said activity knowing said activity may be hazardous to me and my property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTY DAMAGE, OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained by me, or any loss or damage to property owned by me, as a result of being engaged in said activity, WHETHER CAUSED BY NEGLIGENCE OF RELEASES or otherwise.

I further AGREE TO INDEMNIFY AND HOLD HARMLESS THE RELEASES from any loss, liability, damage or costs, including court costs and attorneys’ fees, that they might incur due to my participation in said activity, WHETHER CAUSED BY THE NEGLIGENCE OF RELEASES or otherwise. I hereby further AGREE TO INDEMNIFY AND HOLD HARMLESS THE RELEASES from any loss, liability, damage or costs, including court costs and attorneys’ fees, that they may incur due to any intentional acts of others.

It is my express intent that this Release and Hold Harmless Agreement shall BIND THE MEMBERS OF MY FAMILY AND SPOUSE if any, if I am alive, and my heirs, assigns and personal representative, if I am deceased, and shall be deemed as RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE the above-named RELEASES.

I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be considered in accordance with the laws of the State of California. I understand that I am giving up important rights by contractually agreeing to not sue or make claim against the RELEASES and their affiliates upon whose premises the activity shall take place.

IN SIGNING THIS RELEASE, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Waiver of Liability and Hold Harmless Agreement, understand it and sign it voluntarily as my own free act and deed; no oral representation, statements, or inducements, apart from the foregoing written agreement, have been made.

I am at least eighteen (18) years of age and fully competent; and I execute this Release for full, adequate and complete consideration fully intending to be bound by name.

Waiver and Release of Liability

IN CONSIDERATION OF the risk of injury that exists while participating in ROLLER DERBY PRACTICE (hereinafter the "Activity"); and IN CONSIDERATION OF my desire to participate in said Activity and being given the right to participate in same;

I HEREBY, for myself, my heirs, executors, administrators, assigns, or personal representatives (hereinafter collectively, "Releasor," "I" or "me", which terms shall also include Releasor's parents or guardian if Releasor is under 18 years of age), knowingly and voluntarily enter into this WAIVER AND RELEASE OF LIABILITY and hereby waive any and all rights, claims or causes of action of any kind arising out of my participation in the Activity; and

I HEREBY release and forever discharge MISSION CITY ROLLER DERBY, located at 3400 Calle Real, Santa Barbara, California 93105, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns (collectively "Releasees"), from any physical or psychological injury that I may suffer as a direct result of my participation in the aforementioned Activity.

I AM VOLUNTARILY PARTICIPATING IN THE AFOREMENTIONED ACTIVITY AND I AM PARTICIPATING IN THE ACTIVITY ENTIRELY AT MY OWN RISK. I AM AWARE OF THE RISKS ASSOCIATED WITH PARTICIPATING IN THIS ACTIVITY, WHICH MAY INCLUDE, BUT ARE NOT LIMITED TO: PHYSICAL OR PSYCHOLOGICAL INJURY, PAIN, SUFFERING, ILLNESS, DISFIGUREMENT, TEMPORARY OR PERMANENT DISABILITY (INCLUDING PARALYSIS), ECONOMIC OR EMOTIONAL LOSS, AND DEATH. I UNDERSTAND THAT THESE INJURIES OR OUTCOMES MAY ARISE FROM MY OWN OR OTHERS' NEGLIGENCE, CONDITIONS RELATED TO TRAVEL TO AND FROM THE ACTIVITY, OR FROM CONDITIONS AT THE ACTIVITY LOCATION(S). NONETHELESS, I ASSUME ALL RELATED RISKS, BOTH KNOWN AND UNKNOWN TO ME, OF MY PARTICIPATION IN THIS ACTIVITY.

I FURTHER AGREE to indemnify, defend and hold harmless the Releasees against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney's fees and any related costs.

I FURTHER ACKNOWLEDGE that Releasees are not responsible for errors, omissions, acts or failures to act of any party or entity conducting a specific event or activity on behalf of Releasees. In the event that I should require medical care or treatment, I authorize Mission City Roller Derby to provide all emergency medical care deemed necessary, including but not limited to, first aid, CPR, the use of AEDs, emergency medical transport, and sharing of medical information with medical personnel. I further agree to assume all costs involved and agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance.

I FURTHER ACKNOWLEDGE that this Activity may involve a test of a person's physical and mental limits and may carry with it the potential for death, serious injury, and property loss. I agree not to participate in the Activity unless I am medically able and properly trained, and I agree to abide by the decision of the Missions City Roller Derby official or agent, regarding my approval to participate in the Activity.

I HEREBY ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS "WAIVER AND RELEASE" AND FULLY UNDERSTAND THAT IT IS A RELEASE OF LIABILITY. I EXPRESSLY AGREE TO RELEASE AND DISCHARGE MISSION CITY ROLLER DERBY AND ALL OF ITS AFFILIATES, MANAGERS, MEMBERS, AGENTS, ATTORNEYS, STAFF, VOLUNTEERS, HEIRS, REPRESENTATIVES, PREDECESSORS, SUCCESSORS AND ASSIGNS, FROM ANY AND ALL CLAIMS OR CAUSES OF ACTION AND I AGREE TO VOLUNTARILY GIVE UP OR WAIVE ANY RIGHT THAT I OTHERWISE HAVE TO BRING A LEGAL ACTION AGAINST MISSION CITY ROLLER DERBY FOR PERSONAL INJURY OR PROPERTY DAMAGE.

To the extent that statute or case law does not prohibit releases for ordinary negligence, this release is also for such negligence on the part of Mission City Roller Derby, its agents, and employees.

I agree that this Release shall be governed for all purposes by California law, without regard to any conflict of law principles. This Release supersedes any and all previous oral or written promises or other agreements.

In the event that any damage to equipment or facilities occurs as a result of my or my family's or my agent's willful actions, neglect or recklessness, I acknowledge and agree to be held liable for any and all costs associated with any such actions of neglect or recklessness.

THIS WAIVER AND RELEASE OF LIABILITY SHALL REMAIN IN EFFECT FOR THE DURATION OF MY
PARTICIPATION IN THE ACTIVITY, DURING THIS INITIAL AND ALL SUBSEQUENT EVENTS OF PARTICIPATION.

THIS AGREEMENT was entered into at arm's-length, without duress or coercion, and is to be interpreted as an agreement between two parties of equal bargaining strength. Both I and Mission City Roller Derby agree that this agreement is clear and unambiguous as to its terms, and that no other evidence shall be used or admitted to alter or explain the terms of this agreement, but that it will be interpreted based on the language in accordance with the purposes for which it is entered into. In the event that any provision contained within this Release of Liability shall be deemed to be severable or invalid, or if any term, condition, phrase or portion of this agreement shall be determined to be unlawful or otherwise unenforceable, the remainder of this agreement shall remain in full force and effect. If a court should find that any provision of this agreement to be invalid or unenforceable, but that by limiting said provision it would become valid and enforceable, then said provision shall be deemed to be written, construed and enforced as so limited.

In the event of an emergency, Mission City Roller Derby agrees to contact the person(s) I listed as my emergency contact in the information I provided upon registering for the league or league-organized event.

I, THE UNDERSIGNED PARTICIPANT, AFFIRM THAT I AM OF THE AGE OF 18 YEARS OR OLDER, AND THAT I AM FREELY SIGNING THIS AGREEMENT. I CERTIFY THAT I HAVE READ THIS AGREEMENT, THAT I FULLY UNDERSTAND ITS CONTENT AND THAT THIS RELEASE CANNOT BE MODIFIED ORALLY. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND THAT I AM SIGNING IT OF MY OWN FREE WILL.

Women's Flat Track Roller Derby (WFTDA) Release and Waiver of Liability, Assumption of Risk, and Indemnity Agreement

IN CONSIDERATION of being permitted to participate this date, in any way, at any time, in Women’s Flat Track Roller Derby (“Activity”), I, for myself, my personal representatives, assigns, heirs, and next of kin:

1. ACKNOWLEDGE, agree, and represent that I understand the nature of this Activity, and that I am qualified, in good health, and in proper physical condition to participate in such Activity. I further agree and warrant that if, at any time, I believe the conditions to be unsafe, I will immediately discontinue further participation in this Activity.

2. FULLY UNDERSTAND that: (a) THIS ACTIVITY INVOLVES RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, AND DEATH ("Risks"); (b) these Risks and dangers may be caused by my own actions or inactions, the actions or inactions of others participating in the Activity, the conditions in which the Activity takes place, or THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW; (c) there may be OTHER RISKS or SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation, in the Activity.

3. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE the sanctioning organization(s), their administrators, directors, agents, officers, members, volunteers, and employees, other participants, officials, rescue personnel, sponsors, advertisers, owners and lessees of Premises on which the Activity is conducted, (each of the forgoing shall be considered one of the RELEASEES herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED, OR ALLEGED TO BE CAUSED, IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS; AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my behalf, makes a claim against any of the RELEASEES, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may be incurred as the result of such claim.

I ACKNOWLEDGE THAT I AM OVER THE AGE OF 18 YEARS, HAVE READ THIS AGREEMENT AND FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, HAVE SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE, AND I INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID, THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT.

I further grant the Released Parties the right to photograph and/or videotape and further display, use and/or otherwise exploit name, face, likeness, voice, and appearance forever and throughout the world, in all media, whether now known or hereafter devised, throughout the universe in perpetuity (including, without limitation, in online webcasts, television, motion pictures, films, newspapers, and magazines) and in all forms including, without limitation, publication of Event results and standings, without compensation, reservation or limitation.

The Released Parties are, however, under no obligation to exercise any rights granted herein.

Consent to Use of Electronic Transmissions

In accordance with Corporations Code §§20 and 5079 and the Bylaws of this corporation, the undersigned hereby agrees to the following:

1. The Corporation may send meeting notices, annual reports, and all other materials ("Records") to me by electronic transmission at the e-mail address or facsimile number I provided upon registering for the league or league-organized event.

2. The Corporation may rely on communications sent by me to the corporation by electronic transmission from the e-mail address or facsimile number (if any) listed below for any purposes, including action by written consent. I hereby certify that the Corporation may reasonably conclude that I am the author of communications so sent.

3. I understand that I have the right to have all Records provided or made available on paper or in non-electronic form, but only if requested by me in writing. Requested Records shall be provided within a reasonable period of time.

This consent shall remain in full force and effect until I revoke it in writing and so notify the Corporation.

Media/Model Release Form

For valuable consideration received, I hereby grant to Mission City Roller Derby, or anyone authorized by them, the absolute and irrevocable right and permission, in respect of the photographs, video, or any other form of media, taken of me, or in which I may be included with others, to copyright the same, in their own name or otherwise; to use, re-use, publish, and re-publish, the same in whole or in part, individually or in matter, in any and all media now or hereafter known, and for any purpose whatsoever; and to use my name in connection therewith if he/she/they choose.

I hereby release and discharge Mission City Roller Derby or anyone authorized by them from any and all claims and demands arising out of or in connection with the use of the photographs/video/other media, including without limitation, any and all claims for libel or invasion of privacy.

This authorization and release shall also insure to the benefit of the heirs, legal representatives, licensees, and assigns of Mission City Roller Derby and those authorized by them, as well as the person(s) for whom the photographs were taken.

I am full age and have the right to contract in my own name. I have read and foregoing and fully understand the contents thereof. This release shall be binding upon me and my heirs, legal representatives, and assigns.

Waiver/Release For Communicable Diseases Including Covid-19

ASSUMPTION OF RISK/WAIVER OF LIABILITY/INDEMNIFICATION AGREEMENT

In consideration of being allowed to participate on behalf of MISSION CITY ROLLER DERBY athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (insert name of sports organization) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

Earl Warren Showgrounds Release and Waiver of Liability Agreement (Covid-19)

I, (“Participant”), acknowledge that I will be participating as a volunteer or participant in the following activities at the EARL WARREN SHOWGROUNDS Fair premises and facilities (the “Fair”):

Mission City Roller Derby team practice and related activities
(Roller skating, roller derby, physical contact sport, physical exercise)

I AM AWARE OF THE COVID-19 PANDEMIC AND RELATED GOVERNMENTAL ORDERS, DIRECTIVES AND GUIDELINES (COLLECTIVELY “DIRECTIVES”), INCLUDING DIRECTIVES FOR FREQUENT HAND WASHING, SOCIAL DISTANCING AND USE OF FACE MASKS IN PUBLIC LOCATIONS. I AM AWARE THAT THESE ACTIVITIES ARE OCCURRING IN A PUBLIC LOCATION DURING THE COVID-19 PANDEMIC, AND ALSO POTENTIALLY INVOLVE LIVESTOCK OR OTHER FAIRGROUNDS ACTIVITIES, AND ARE THEREFORE HAZARDOUS ACTIVITIES. I AM AWARE THAT I COULD BE INFECTED, SERIOUSLY INJURED OR EVEN DIE DUE TO COVID-19 OR DUE TO ACTIVITIES ON THE FAIRGROUNDS INCLUDING BUT NOT LIMITED TO LIVESTOCK ACTIVITIES. I AM VOLUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH KNOWLEDGE OF THE DANGER INVOLVED AND AGREE TO ASSUME ANY AND ALL RISKS OF BODILY INJURY, DEATH OR PROPERTY DAMAGE, WHETHER THOSE RISKS ARE KNOWN OR UNKNOWN.

As consideration for being permitted by the Fair, the State of California (“State”), the County of Santa Barbara (the “County”), and any lessor of the fair premises (“Lessor”), to participate in these activities and use the Fair premises and facilities, I forever release the Fair, the State, California Fair Services Authority (“CFSA”), the County, the Lessor, and any fair affiliated organization, along with their respective directors, officers, employees, volunteers, agents, contractors, and representatives (collectively “Releasees”) from any and all liabilities, causes of action, lawsuits, claims, demands, or damages of any kind whatsoever that I, my assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives now have, or may have in the future, for injury, death, or property damage, related to (i) my participation in these activities, (ii) the negligence or other acts of any Releasee, whether directly connected to these activities or not, and however caused, or (iii) the condition of the premises where these activities occur, whether or not I am then participating in the activities. I also agree that I, my assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives will not make a claim against, sue, or attach the property of any Releasee in connection with any of the matters covered by the foregoing release.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND THE FAIR, THE STATE, CFSA, THE COUNTY, AND THE LESSOR, AND SIGN IT OF MY OWN FREE WILL.

Covid-19 Protocol and Reporting Guidelines

To participate in moderate- and high-contact sports practice or competition, participants must commit to adherence with appropriate isolation and quarantine procedures.

  • All symptomatic athletes and staff must remain in isolation until test results are available, and repeat testing should be considered if initial testing is negative and symptoms continue and are consistent with COVID-19 symptoms.
  • All athletes and staff who test positive or are clinically diagnosed with COVID-19 disease must isolate:
    • For 10 days after symptoms first appeared (or 10 days after specimen collection for their first positive test), AND
    • At least 24 hours have passed with no fever (without use of fever-reducing medications), AND
    • Until other symptoms have improved.
  • Individuals who test positive for SARS-CoV-2 but never develop symptoms, may return to practice 10 days after the date of specimen collection for their first positive test for SARS-CoV-2.
  • Individuals identified as close contacts (those who have had exposure to a COVID-19 positive individual within 6 feet for at least 15 minutes) must follow state and local public health directives or orders for quarantine, regardless of the frequency of testing for those in the cohort or footprint. Close contacts must monitor their health and abstain from contact drills for at least 5 days after first exposure.
  • Due to the nature of athletic participation, teams must take steps to assist with the contact tracing process including, but not limited to, film review of practice, if available, and extended assessment of contact associated with athletic participation so that all athlete contacts can be identified.

In-Practice Protocol
All participants must:

  • Sign a waiver of liability specific to COVID-19 & Earl Warren Showgrounds (required by EWS)
  • Sign an agreement to abide by all MCRD & Earl Warren COVID-19 rules at all times
  • Alert the team manager if exposed to COVID-19 or received a positive test result within 10 days of attending an in-person practice
  • In-Person Practices Will Be:
    • If unvaccinated, skaters must wear a mask and maintain 6ft social distancing when attending practice. Unvaccinated skaters must abstain from contact drills.
    • If vaccinated, skaters are not required to wear a mask while skating if able to maintain 6ft social
      distancing.
    • All skaters are encouraged to wear a mask during contact drills.

Reporting Procedure:

  1. Contact the team manager and give them the pertinent details
    • Include:
      • Last date at practice, if known
      • Date symptoms started (approximate if unsure)
      • Date of positive COVID-19 test or last exposure as a close contact
  2. Follow CDC established guidelines and your PCP recommendations for quarantining and home isolation. Update the team manager on your progress (because we worry about you).
  3. Return to practice when you meet the following criteria:
    • If you’ve tested positive for COVID-19:
    • If symptomatic: you have completed a 10-day isolated quarantine after your symptoms first
      appeared
    • If symptomatic: 24 hours have passed with no fever without the use of fever-reducing medications and other symptoms have improved
    • If asymptomatic: you have completed a 10-day isolated quarantine after specimen collection for
      your first positive test and you have not developed symptoms of COVID-19
    • If you are a close contact of an individual who has tested positive for COVID-19:
      • You have not developed symptoms of COVID-19 within 5 days of first contact with the individual
      • You have not tested positive for COVID-19 within that time

Please Note:

  • MCRD will not tolerate any harassment, discrimination, or bullying of participants who are exposed to or test positive for COVID-19
  • MCRD will strictly enforce all COVID-19 rules and guidelines. Participants who knowingly break these rules at in-person practices will be asked to leave practice immediately and potentially subject to sanctions following a review by the MCRD Board of Directors
  • If a participant who has been at an in-person practice tests positive for COVID-19, team leadership will:
    • Immediately suspend in-person practice for an amount of time in accordance with CDC guidelines
    • Notify every participant present at recent in-person practices and events of potential exposure via text message and email notifications
    • Maintain the privacy and confidentiality of individuals who report exposure or a positive COVID-19 test result to the Team Manager

Electronic Signature Consent

I consent to the use of my electronic signature in lieu of an original signature on paper. I have the right to request that I sign a paper copy instead. I understand that I may, upon written request to MCRD, obtain a paper copy of an electronic record and that, by checking the box below, I waive that right. I understand that no fee will be charged for such copy and no special hardware or software is required to view it. My agreement to use an electronic signature with MCRD for any documents will continue until such time as I notify MCRD in writing that I no longer wish to use an electronic signature. I understand that there is no penalty for withdrawing my consent. I understand that I am responsible for making sure MCRD has my current email address on file in order to contact me regarding any changes, if necessary. 

I consent to provide my electronic signature in order to agree that I understand and agree to the information outlined in all above sections. I understand that an electronic signature will hold the same weight in court as a standard signature written by hand.
